Scott Adams may not have much longer left to live amid his battle with prostate cancer.

The “Dilbert” creator, 68, is reportedly in hospice at his home in Northern California, his first ex-wife, Shelly Miles, told TMZ on Monday.

Miles, who was married to the legendary cartoonist from 2006 to 2014, also informed the outlet that Adams began receiving “end-of-life care” at his home last week because his health was declining “rapidly.”

Although nurses have reportedly been “coming in and out” around the clock, Miles said that she, her sister and the “Dilbert” creator’s stepdaughter, Savannah, have been helping to care for Adams in his final days.

Miles’ claim that Adams may not have much longer to live comes less than a year after the cartoonist and author announced he was diagnosed with metastatic prostate cancer in May 2025.

Adams indicated that he had been dealing with his condition privately for some time before his public announcement, but noted that the cancer had progressed to Stage 4 and spread to his bones.

He then shared another harrowing health update earlier this month.

“I talked to my radiologist yesterday, and it’s all bad news — the odds of me recovering are essentially zero,” he said during his “Real Coffee with Scott Adams” podcast on Jan. 1. “I’ll give you any updates if that changes, but it won’t.

“So there’s no chance that I’ll get my feeling back in my legs, and I’ve got some ongoing heart failure, which is making it difficult to breathe sometimes during the day,” the iconic artist continued. “However, you should prepare yourself that January will probably be a month of transition, one way or another.”

Adams then explained that he had not yet “made any decisions” regarding his worsening condition.

“But it was all bad news, no good news at all,” the artist repeated. “So I will keep doing this as long as it makes sense — because I like doing it, it keeps me busy.”

“I have much bigger problems than the stuff I’m talking about in the news,” Adams, who revealed he was still working on new comics amid his cancer battle, added. “But I’m so interested in like what’s happening in the world.”

Adams created “Dilbert” in 1989.

In 2023, however, more than 80 newspapers stopped printing the cartoon after the cartoonist was accused of spouting racist remarks.

Adams had said on his podcast that black people were “a hate group” and urged white people to “just get the hell away” from them.

Despite the controversy, Adams continued to host his podcast and promote his cartoon.
